Missed Call Cost Calculator for PT and Chiropractic Clinics

Short answer

A clinic that gets 120 calls a month and misses 30% of them loses about 7 new patients a month if 40% of missed callers are new and half would have booked. At a $3,000 plan of care, that is $21,600 a month.

The number most owners have never calculated is what the phone costs them when nobody picks up. Ads, referrals and Google all end in the same place, a phone ringing at your front desk, and if that call goes to voicemail at 5:40pm on a Tuesday the marketing worked and the clinic still lost the patient. The calculator below uses the same math we run on the Missed Patient Test with clinics. Every default is editable; change them to match your call log.

How does the missed call calculation work?

The calculator multiplies five numbers: calls per month, the share you miss, the share of missed calls that are new patients, the share of those who would have booked, and what one new patient is worth. The defaults (120 calls, 30% missed, 40% new, 50% would book, $3,000) are the assumptions we start with on a call before we have the clinic's own log. They are not industry averages. A clinic with one front desk person who also checks people out and takes payments usually misses more than 30% during the busy hours; a clinic with a dedicated phone line and evening coverage misses less. The useful part is not the default answer. It is replacing each default with your own number from your phone system and seeing which one moves the total the most.

Why do clinics miss so many calls?

Clinics miss calls for boring reasons: the front desk is checking someone out, the phone rings during lunch, it is after 5pm, or it is Saturday. The calls that matter most tend to arrive at exactly those times, because a working adult with shoulder pain calls on a break or after work. That is why the Missed Patient Test we run with clinics is built around three moments: a call at 5:40pm on a Tuesday, a call at 10:15am on a Saturday, and a web form submitted at 6:10pm on a Friday. If those three get handled, the rest of the week usually does too. None of this is a staffing failure. It is a system that was built for patients who already have an appointment, not for new ones who are deciding which clinic to call.

What is a new patient worth to a cash-based clinic?

A new patient is worth their first plan of care, not their first visit. If your ad sells a $79 evaluation and the patient goes on to buy a $1,750 package of ten visits, the missed call cost you $1,750, not $79. For reference, Rehab Lab, a two-location cash PT client in Wisconsin, sells packages from $5,000 for 12 visits, and their shockwave program carries a $2,850 plan of care. A chiropractic office with a typical case of 12 to 20 visits plus a maintenance plan will land somewhere different. Put in your own average. If you do not know it, divide last quarter's collections from new patients by the number of new patients, and use that.

How much does this usually come to?

With the defaults the answer is about 7.2 lost new patients and $21,600 a month, which is $259,200 a year. Owners usually react in one of two ways. Some say the number is too high, and they are often right about one input, typically the booking rate, so they drop it to 30% and the total falls to about $13,000. That is still more than most clinics spend on ads. The other reaction is quieter: they pull the call log, count, and find they were missing more than 30%. Either way the exercise does its job, which is turning an invisible loss into a number you can compare with what fixing it would cost.

How do I cut the number of missed calls?

Start with what fires automatically, then add coverage. In order of cost:

  1. Missed-call text back. When a call goes unanswered, the caller gets a text within a minute: "Sorry we missed you, this is the front desk at your clinic, what can we help with?" Most phone platforms and CRMs, GoHighLevel included, can do this out of the box.
  2. Answer the forms in five minutes. Web form and Facebook lead form submissions get a text and an email within five minutes, nights and weekends included. See speed to lead for clinics.
  3. Cover the gaps. Lunch, after 5pm and Saturday mornings get an answering service or an AI receptionist that can book into your calendar.
  4. Script the call. A caller who reaches a person still needs to be booked. Use a new patient phone script built for cash pay.
Our takeFix the phone before you raise the ad budget. More ads into a phone that misses a third of its calls just buys more voicemails.

How does this compare with what I spend on ads?

Compare the monthly missed-call number with your monthly ad spend and your cost per new patient. If you pay $150 to $300 to acquire a new patient through ads, as many clinics do (see what a new patient costs by channel), every missed new-patient call is that money spent and lost. A clinic losing seven new patients a month to the phone is effectively paying for those seven twice: once in ad spend to make the phone ring, and once in the revenue that went to the clinic down the street. That is why the first thing we look at with a new coaching client is almost never the ad.

Questions owners ask

How do I find out how many calls my clinic misses?

Pull the call log from your phone system for the last 30 days and count calls with no answer, calls that went to voicemail, and calls under 10 seconds. Most VoIP systems, and GoHighLevel's phone, show this in one report.

Do all missed callers call back?

No. Some leave a voicemail and some call again, but a new patient who reaches voicemail often calls the next clinic on the list. That is why the calculator only counts the share who would have booked, not every missed call.

What counts as the value of a new patient?

Use what one new patient is worth to you over their first plan of care or first year, not the price of the first visit. For a cash PT clinic that is usually the package price; for a chiropractic office it is the average case value.

What is the fastest fix for missed calls?

A missed-call text that goes out automatically within a minute, followed by a person or an AI receptionist who answers after hours. The text alone catches a share of callers; answering the phone catches more.
