What Is Collections per Visit? Definition and Formula for Clinics

Definition

Collections per visit is the average money a clinic actually collects for each patient visit, calculated as total collections divided by total patient visits over the same period.

Collections per visit is how much money actually lands in the bank for each patient visit, on average. It is a cleaner number than your fee schedule because it includes discounts, write-offs and unpaid balances. Take Rehab Lab, a cash PT client whose most common package is $5,000 for 12 visits: that works out to about $417 collected per visit (our arithmetic: $5,000 / 12). An in-network clinic billing the same visit would see a fraction of that.

How do you calculate collections per visit?

Collections per visit = total collections / total patient visits, over the same period. Use money actually received, not amounts billed, and count every visit, including follow-ups. Worked example: a chiropractic office collected $54,000 last quarter across 600 visits, so $54,000 / 600 = $90 per visit. For package clinics, collections and visits can fall in different months, since the patient pays up front. Use a quarter or longer to smooth that out.

Why use collections instead of charges?

Use collections because charges can look healthy while the bank account does not. Rehab Lab's October 2025 is a good illustration: about $110,000 invoiced against $72,000 collected that month. Owners who track invoiced revenue alone overestimate what each visit earns, and then overestimate what they can spend on ads. In an insurance practice the gap includes contracted rates, denials and slow payment. In a cash clinic it is mostly timing on payment plans. Either way, collections is the number your profit margin actually comes from.

How does collections per visit differ for cash and insurance clinics?

Cash clinics usually collect more per visit, because they set their own price and get paid at the visit, while insurance clinics accept the plan's allowed amount and wait. Insurance clinics often see more visits per week to make up the difference. The useful comparison is collections per visit times visits per hour, which shows what each hour of clinician time earns. The cash vs insurance calculator runs that comparison, and collections per visit feeds directly into patient lifetime value.

Questions owners ask

What is the difference between collections and charges?

Charges are what you billed; collections are what landed in the bank. In an insurance clinic the gap can be large because of contracted rates, denials and slow payment. In a cash clinic the two are close.

How do packages affect collections per visit?

Divide the package price by the visits in it. A $1,500 package for 10 visits is $150 per visit, even though the patient paid once.

Should I include no-show fees and retail sales?

Keep them out of collections per visit so the number reflects care, and track them separately. Whatever you choose, use the same rule every month.
