The Longest-Running Clinic Ads: Chiro and PT Ad Examples
The longest-running clinic ads in our 18-metro pull were live for 25 to 45 months. They share four traits: a specific offer, a local name or place, a real patient testimonial, or an invitation to an event like a seminar. Tenure is a signal, not proof the ad is profitable.
When we pulled the Meta Ad Library for 18 US metros on October 1, 2026, we logged 1,046 live clinic ads from 312 clinics. A handful had been running for two, three, almost four years. The oldest, from a chiropractic office in West Palm Beach, went live on December 29, 2022. Ads that survive that long are worth studying because clinic owners and agencies tend to turn off what does not bring patients in. They are not proof of profit, and this page says plainly where the evidence stops.
What are the longest-running clinic ads in 18 metros?
The longest-running clinic ads in our October 2026 pull ran from 11 to 45 months, led by a West Palm Beach chiropractic ad at 45 months and a Chapel Hill PT ad at 42. The table lists the 12 oldest live ads we found among 312 advertising clinics, with the headline as it appeared in the Meta Ad Library. Chiropractic offices hold 6 of the 12 spots, close to their share of advertisers (172 of 312). Only two of the 12 put a price in the headline, both $49. "Months" is the time from the start date to our pull date. One row tagged PT in our data is a podiatry ad, which we left in as logged.

What do the longest-running clinic ads have in common?
The longest-running clinic ads have at least one of four things in common: a specific offer, a local name or place, a real patient testimonial, or an event invitation. The Cedar Park $49 special (40 months) and the Tampa $49 wave therapy offer (19 months) lead with a price for a defined visit. The West Palm Beach ad (45 months), "Cedar Park Friends" (18 months) and "Pain Specialists of Frisco" (16 months) lead with the place. Fort Worth's pediatric testimonial (25 months) lets a parent do the selling. Encinitas runs a free chronic pain seminar (33 months). What is missing is just as telling: none of the 12 is a clever concept, a stock-photo brand campaign, or a list of every condition the clinic treats. They are plain. A stranger knows within a second who the clinic is, where it is, and what to do next.
| Pattern | Examples from the top 12 | Months live |
|---|---|---|
| Specific offer | Cedar Park $49 special; Tampa $49 wave therapy | 40; 19 |
| Local name or place | West Palm Beach clinic name; Cedar Park Friends; Pain Specialists of Frisco | 45; 18; 16 |
| Testimonial | Fort Worth pediatric chiropractic | 25 |
| Event or seminar | Encinitas free chronic pain seminar | 33 |
| Problem or alternative | Fort Worth disc pain; Phoenix pain management | 22; 25 |
| Services and sports | Chapel Hill dry needling and sports performance | 42 |
Why does a specific offer last longer than a general ad?
A specific offer lasts longer because it gives a stranger one clear, low-risk next step, and that keeps working long after a clever headline wears out. The Cedar Park chiropractic office has run its "$49 Special Offer" since May 26, 2023, making it 40 months old at our pull and the oldest priced ad in the data. Across all 18 metros, 122 of 312 clinics priced their first visit, the median was $49, and $49 was the most common price at 34 clinics. So the price is not what made the Cedar Park ad unusual. Staying power did. An offer that converts does not need to be replaced every month, and owners who keep chasing new creative often abandon something that was working. Look up the offers in Cedar Park and the rest of the metro in the Austin local ad report.
Why do local ads with a town name run so long?
Local ads with a town or clinic name run long because they tell the viewer immediately that this is a real place nearby, which is the first filter anyone uses when choosing a clinic. In our October 2026 data, 67 of 312 clinics put the town name in a headline, the second most common angle after a priced first visit. Three of the 12 longest-running ads lead with a place: the West Palm Beach clinic (45 months), the Cedar Park shoulder ad that opens "Cedar Park Friends" (18 months), and the Frisco pain specialists (16 months). The West Palm Beach ad is the extreme case. Its headline is simply the clinic's name and location. That is a brand ad, and brand ads often run cheaply in the background for years. See the clinics running in South Florida.
Why do testimonial ads run so long?
Testimonial ads run long because a real patient's story does not go stale the way an offer or a seasonal hook does, and it answers the question every new patient has: did this work for someone like me? The Fort Worth pediatric chiropractic testimonial in our data has been live since September 6, 2024, 25 months at our pull. Pediatric care is a good fit for the format, because a parent deciding about a child trusts another parent more than any claim the clinic makes. Testimonials also stay inside ad policy more easily. Meta's Personal Attributes policy bars ads that assert or imply the viewer's medical condition (Meta Transparency Center); a patient describing their own experience avoids that trap. Get written permission before you run one. The Dallas-Fort Worth ads are in the Dallas local ad report.
Why does a seminar ad keep running for years?
A seminar ad keeps running because it offers a free, low-pressure first step for people who are not ready to book a visit, and the event can repeat on a schedule. The Encinitas chiropractic office in our data has run "FREE Chronic Pain Seminar" since January 15, 2024, 33 months at our pull. A person with years of chronic pain is often skeptical of another adjustment or another special, and a seminar lets them meet the doctor, hear the explanation and decide in a room with other people. One detail is worth noting honestly: the headline we logged names a specific date, "January 23 at 5:15 PM." We cannot tell from the Ad Library whether the office updates that date in the copy or whether the ad is running with an old date. Check the San Diego ads in the San Diego local ad report.
What about the brand and service-list ads in the top 12?
The brand and service-list ads in the top 12 show that a plain description of the clinic can run for years, probably at low spend, as a steady background presence. The Chapel Hill PT ad, live since March 29, 2023 and 42 months old at our pull, lists what the clinic does: dry needling and sports performance. The La Jolla PT ad offers a facility tour. The Phoenix pain management ad invites a consultation. None of these has a hook in the usual sense, and we would not build a new-patient campaign around any of them. They work as evidence that some clinics keep a brand ad running permanently next to their offer ads. That is a reasonable setup if the brand ad is cheap. The Chapel Hill ad and the rest of the area are in the Raleigh-Durham local ad report.
Does a long-running ad prove that it works?
A long-running ad does not prove it works; tenure is a signal, not proof. Owners and agencies tend to turn off ads that do not bring in patients, so an ad that survives for years probably earns its keep. But there are other reasons an ad can run that long. It might be a cheap brand ad spending a few dollars a day. The clinic might have stopped checking the account. The ad might be one of many in an account where the real budget goes elsewhere. Our pull recorded start dates, headlines, offers and prices. It did not record spend, leads or patients for any of these clinics. Use the list to spot formats worth testing in your market, then judge your own version on cost per qualified lead and cost per new patient.
How can I find the longest-running ads in my own market?
You can find the longest-running ads in your own market by searching the Meta Ad Library for your town plus "chiropractor" or "physical therapy" and checking the start date on each ad. The Meta Ad Library is Meta's public, searchable database of ads running across Facebook and Instagram, where each ad shows its creative and the date it started running. Note the start date, the offer, the price and the format of anything that has run longer than six months. Do this for 10 to 20 competitors and a pattern usually shows up within an hour. Our glossary entry on the Meta Ad Library explains the search. Or skip the work: our free local ad report does this for your metro, and the 2026 clinic advertising study has the 18-metro totals.
Questions owners ask
What is the longest-running chiropractic ad in your data?
A West Palm Beach chiropractic, acupuncture and rehab clinic's ad, live since December 29, 2022, which made it 45 months old when we pulled the Meta Ad Library on October 1, 2026.
Does a long-running ad mean it is profitable?
Not necessarily. Advertisers usually keep paying for ads that bring in patients, so tenure is a useful signal, but an ad can also run because it is a cheap brand ad or because nobody turned it off. The Ad Library does not show us results.
Where can I see the ads running in my city?
Search the Meta Ad Library for your town plus chiropractor or physical therapy, or request our free local ad report, which lists the clinics, offers and prices running in your metro.
Should I copy a competitor's long-running ad?
Copy the structure, not the words. If a $49 offer or a testimonial format has lasted years in your market, test the same format with your own offer, your own patients and your own town name.
