GoHighLevel for Chiropractors and Physical Therapy Clinics

Short answer

GoHighLevel is a CRM and marketing platform that handles a clinic's leads, missed-call texts, forms, calendars, follow-up workflows, reviews and AI chat. It is not an EMR and is not HIPAA compliant by default; plans start at $97 a month, and the HIPAA add-on with a BAA is $297 a month.

GoHighLevel is the tool we build in every day, for our own funnels and inside clinics' accounts, so this page is written from the build side, not the brochure. In short: it is very good at the part of a clinic most EMRs ignore, the people who have not booked yet. It catches the missed call, answers the 9pm form, runs the follow-up and asks for the review. It is not a place for clinical notes, and it is not HIPAA compliant until you pay for and switch on the add-on. Whether it is right for you depends on two things: how many leads you get a week, and whether someone at the clinic will work out of it daily.

What does GoHighLevel do for a clinic?

GoHighLevel gives a clinic one place to capture leads, text and call them, book them, follow up automatically and ask for reviews. GoHighLevel (also called HighLevel or GHL) is a CRM and marketing automation platform: contacts, a phone and text inbox, forms, calendars, pipelines, workflows, landing pages, email, review tools and AI chat in one account. For a clinic, that covers everything between "someone saw your ad" and "they showed up for their first visit," plus the reactivation and review work after the plan of care ends. It does not hold the clinical record. The way we set it up, the EMR owns patients who booked and their notes; GoHighLevel owns leads, conversations and marketing. The table maps each feature to the clinic job it does.

How does missed-call text back work in GoHighLevel?

Missed-call text back in GoHighLevel is a workflow that watches your clinic number and sends a text to any caller whose call goes unanswered, usually within a minute. The text should sound like the front desk, not a robot: "Hi, this is the front desk at [Clinic name], sorry we missed your call. What can we help you with?" The reply lands in the same inbox as every other conversation, so the front desk can pick it up from the desktop or the mobile app. We add two things most default setups skip: an internal notification to the front desk so a person follows up, and a filter so existing patients calling to reschedule do not get a sales-style message. It is the cheapest fix in the whole system. To see what missed calls cost you, use the missed call cost calculator.

How do forms, calendars and workflows fit together in a clinic setup?

Forms collect the lead, the workflow responds and follows up, and the calendar is where the lead becomes a booked visit; the pipeline shows where everyone is. A clean setup for a cash clinic looks like this. Each ad offer gets its own form with three to five fields and one qualifying question, such as "what is the main thing you want help with?" A workflow fires on submission: text within a minute, email right after, internal alert to the front desk, and a follow-up text the next morning if nobody booked. The calendar shows real availability with buffers so the front desk is not double-booked. Every lead moves through pipeline stages (new, contacted, booked, showed, bought a plan) so the owner can see the leak in one screen. Five-minute replies are the target; see speed to lead for clinics.

Can GoHighLevel get a clinic more Google reviews?

GoHighLevel can get a clinic more Google reviews by sending a review request by text automatically after a visit and letting you reply to reviews from the same account. The timing matters. We trigger the request after a set visit number in the plan, or when the front desk tags a patient who just reported a win, rather than after the first visit when the patient has not seen results yet. BrightLocal's Local Consumer Review Survey 2026 found that 74% of consumers only care about reviews from the last three months and 47% would not use a business with fewer than 20 reviews (BrightLocal, Feb 2026). That makes a steady weekly flow worth more than a one-time push, and automation is how you keep it steady. HighLevel's HIPAA article notes that Reviews AI replies follow its HIPAA handling for HIPAA-enabled sub-accounts.

What can GoHighLevel's AI do for a clinic?

GoHighLevel's AI can answer and qualify leads by text and chat, and with voice AI answer calls, then book into your calendar, which covers the hours your front desk is not there. In practice the AI is only as good as what you give it: your offer, prices, hours, what you treat and do not treat, how to answer "do you take insurance," and when to hand off to a person. Rehab Lab, a two-clinic cash PT client, had a bad month in May 2026 when Facebook optimized for message volume instead of quality and cost per qualified lead rose from about $11 to about $17. The fix was a qualifying question, then landing pages and an AI setter built in Rehab Lab's own GoHighLevel account. HighLevel's pricing page lists premium AI usage as billed separately (HighLevel pricing). For the receptionist side, see AI receptionist for clinics.

What does GoHighLevel not do for a clinic?

GoHighLevel does not do clinical work: it has no clinical documentation, no plans of care, no home exercise programs and no insurance claims, so it does not replace an EMR. It also does not fix a weak offer or a front desk that ignores the inbox. The tools only work if someone opens the conversations every day and calls the leads the automation flagged. And it is not HIPAA compliant out of the box, which is the next section. The right model is two systems: an EMR such as Jane, WebPT or Practice Better for patients who booked, and GoHighLevel for leads and marketing. Keep diagnoses and treatment details out of GoHighLevel where you can; you rarely need them to follow up a lead. Our EMR comparison for cash clinics covers the other half.

Is GoHighLevel HIPAA compliant for a clinic?

GoHighLevel is not HIPAA compliant by default; HighLevel sells a HIPAA add-on that includes a business associate agreement, and you have to buy it and switch it on. HighLevel's help center states that "HighLevel accounts are NOT HIPAA compliant by default," that the add-on costs $297 a month, includes ePHI encryption, a BAA, audit logging and MFA enforcement, and that "once enabled, HIPAA is permanent for your agency and cannot be canceled, refunded, removed, or downgraded" (HighLevel Help). After purchase, the agency owner must enable HIPAA on each sub-account. HighLevel's HIPAA page adds that the customer "is ultimately responsible for meeting all HIPAA requirements" (HighLevel). HHS says covered entities generally need contracts with business associates that handle protected health information (HHS). If you use an agency's account, ask whether they have the add-on and will sign a BAA with you.

How much does GoHighLevel cost for a clinic?

GoHighLevel costs $97 a month on its Starter plan, $297 on Unlimited and $497 on Agency Pro, with a 14-day free trial, plus usage charges for phone, text, email and premium AI, plus $297 a month if you add HIPAA (HighLevel pricing, checked October 23, 2026). Annual billing is $970, $2,970 and $4,970. A single clinic does not need Unlimited or Agency Pro; those plans exist for agencies running many sub-accounts. Starter includes up to three sub-accounts, which covers one or a few locations. HighLevel also lists an Enterprise plan with custom pricing that includes HIPAA. The usage charges are small per message but add up with large reactivation sends, so budget for them.

How long does a GoHighLevel clinic setup take?

A working GoHighLevel setup for one clinic takes a day or two for the basics and two to four weeks to get the follow-up and front desk habits right. The first day covers the phone number, missed-call text back, one calendar, one form for your main offer, the five-minute reply workflow and review requests. Weeks two to four are where the value is: pipeline stages that match how your clinic actually sells, a no-show follow-up, a 90-day reactivation workflow, an email to past patients, and training the front desk to clear the inbox several times a day. The most common mistake we see is building 40 automations before the front desk opens the inbox once. Build the five that matter, run them for a month, then add.

Setup stepTimeNotes from our builds
Phone number and inboxDay 1Keep your existing number if patients know it
Missed-call text backDay 1Exclude existing patients from sales wording
Calendar and remindersDay 1Real availability and buffers only
Offer form and five-minute workflowDay 1 to 2One qualifying question on every form
Review requestsDay 2Trigger after a set visit number, not visit one
Pipeline stagesWeek 2New, contacted, booked, showed, bought
Reactivation workflowWeek 2 to 390-day recall, two texts, call task
Front desk trainingWeeks 2 to 4Inbox cleared several times a day

Should a clinic own its GoHighLevel account or use an agency's?

A clinic should own its GoHighLevel account, or at least own a sub-account it can take with it, because the leads, conversations, workflows and phone number are business assets. This is the "owned" part of the way we work: everything lives in the clinic's own accounts, so if the clinic and the person building it part ways, the system stays. Rehab Lab's landing pages and AI setter were built in Rehab Lab's own GoHighLevel account for exactly that reason; read the Rehab Lab case study for the numbers. If an agency runs it for you, get it in writing that the sub-account, number and data transfer to you, and confirm the HIPAA add-on and BAA cover your sub-account. HighLevel notes HIPAA sub-accounts can only move between agencies that both have HIPAA enabled.

Our takeGoHighLevel is worth it for a clinic the day it starts running ads, and not much before. The setup that matters is small: missed-call text back, a five-minute reply, a calendar, review requests and a reactivation workflow. Everything else can wait.

This is general information, not legal or billing advice; check your state board and a healthcare attorney before you change how you bill.

Questions owners ask

Is GoHighLevel HIPAA compliant?

Not by default. HighLevel's help center says accounts are not HIPAA compliant by default; its HIPAA add-on costs $297 a month, includes a BAA, must be enabled for each sub-account, and cannot be cancelled once turned on.

Can GoHighLevel replace my EMR?

No. GoHighLevel has no clinical documentation, plans of care or insurance claims. Use it next to an EMR such as Jane or WebPT for the marketing and follow-up side, and keep clinical notes in the EMR.

How much does GoHighLevel cost for a chiropractor?

HighLevel's pricing page lists Starter at $97 a month, Unlimited at $297 and Agency Pro at $497, each with a 14-day trial. Phone, text, email and premium AI usage are billed separately, and HIPAA is a $297 monthly add-on.

What is missed-call text back in GoHighLevel?

It is an automation that sends a text to anyone whose call to your clinic goes unanswered, usually within a minute, so the caller can reply by text instead of calling the next clinic. In GoHighLevel it is a simple workflow on your clinic number.

Is GoHighLevel hard to set up for a clinic?

The basics, a number, a calendar, a form and a missed-call text, take a day or two. The parts that take longer are the follow-up workflows, the pipeline stages and training the front desk to work out of it every day.
